By taking a team approach to dialysis care management, Dialysis At Home can achieve the best possible outcomes, the most efficient use of resources, and continuous client satisfaction and involvement.

SERVICES PROVIDED:

STAFF ASSISTED HOME HEMODIALYSIS

An experienced RN, LPN, or Patient Care Technician will come to your house to provide you with your dialysis treatments. He or she will be responsible for all aspects of your care from drawing lab work to ordering your supplies.

Some insurance cover the cost of the caregiver. There is also the option of paying privately for a caregiver to come to your home. All insurances will cover the cost of the equipment & the supplies needed to perform dialysis in your home.

You will still need dialysis 3 days a week, either Mon-Wed-Friday or Tuesday-Thursday-Saturday. With home dialysis you can pick what time of day you would like your treatment. Most clients choose either an early morning slot or an afternoon slot.

The obvious benefit is that you no longer have to go to dialysis clinic, it comes to you! No more worries about driving to a unit in severe weather! You will be able to dialyze in the comfort of your own home. Home dialysis also results in a more consistent care and treatment process. Since you usually have the same caregiver every treatment there is more continuity of care resulting in improved clinical outcomes.

No training is necessary with staff assisted home dialysis, we provide an experienced caregiver to provide you with your treatments. If you qualify for home dialysis, one of our team members will visit your home to evaluate the environment. What you will need is a room to perform your treatments and an appropriate water source. You will need enough storage space to accommodate a month’s worth of dialysis supplies.

A month of home hemodialysis supplies will take up about 40 cubic feet somewhere in your home. So, you could store them in a small closet or in two bookcases. Either way, you’ll need shelves or drawers to be sure you have easy access to everything you need.

With the help of your training nurse, your doctor will prescribe the dialysis therapy that works best for you. How frequently you will dialyze, how many liters of dialysate you will use each treatment along with how long the treatment will last. Most physicians using the System One have taken advantage of the simplicity and flexibility of the system to prescribe short daily dialysis therapy (5 or more times per week). Treatment times are variable, based upon your size, access type and physician prescription. Typically, “short daily” treatments will involve 15-20 liters of dialysate and will last approximately 2.5 hours per session.

Personal Dialysis, Inc. has developed a program to train you and your partner in this home-care dialysis therapy. During this program, you will learn how to access your blood, operate the NxStage system, monitory your vital signs, administer your drugs, and handle emergency situations.

Our specially trained nurses will adapt the program to meet your needs. Training takes place 5 days per week and last between 4-6 hours per session. You will generally complete training in 3 to 4 weeks.

Your partner also participates in training. You are encouraged to do as much of the therapy as you can, and your partner does not need to attend every day of the training program.

Our training nurse can meet with you and your partner to provide you additional information and determine if this therapy is right for you.

PERITONEAL DIALYSIS

Peritoneal dialysis is a therapy that uses the peritoneal membrane (membrane that surrounds the organs in the abdomen) as a dialysis membrane.

There are two types of peritoneal dialysis:

CAPD (Continuous Ambulatory Peritoneal Dialysis): Performed four to five times per day through manual exchanges done by the patient without a machine.

CCPD (Continuous Cycler-Assisted Peritoneal Dialysis): Done in addition to manual day-time exchanges or as the only therapy, CCPD uses an assist machine, called a cycler, to the exchanges, usually while the patient sleeps at night.

Patients are taught to perform the therapy either alone or in combination with a partner. A partner is not required, however. A thorough training course is completed over a period of two to three weeks or longer as needed by the individual learner. The learner is taught all aspects of self-care from performing the exchange procedure to monitoring their response to dialysis therapy as well as ordering/maintaining their home supplies.

Space in the home is required to store a month’s supply of dialysate and associated PD supplies. A closet or about 40 cubic feet of empty space should be adequate.

Traveling/vacationing patients can request that their supplies be delivered to the location they will be staying making travel more feasible and the need for an open spot in a dialysis unit a non-issue.

Peritoneal dialysis patients are seen once a month for a clinic visit by a trained dialysis nurse, their doctor, a dietician and a social worker. Nurses are available 24 hours a day, seven days a week to help manage your home care.

LONG TERM CARE SETTING

LTC facility patients may be transported to Clinics for treatment. Transporting LTC facility patients to off-site dialysis facilities is burdensome to both the LTC facility and the patient. LTC patients are often frail, sensitive to temperature variations, and uncomfortable while being transported to dialysis facilities. Patients routinely miss meals, medications, rehabilitation services, resident activities, and other services provided by the LTC facility due to the length of dialysis treatment, time associated with preparing for transfer to and from the dialysis facility, and actual transportation time. DAH can work with the LTC facility so as to provide the dialysis treatment in the LTC facility. With the treatment being done in house the patient and LTC facility are relieved of all the above issues.
