The
Dialysis At Home care-management team is made up of a nephrologist (kidney
doctor), an RN/Dialysis Care Manager, a Home Caregiver, a Renal Social
Worker, a Renal Dietitian, an Equipment Technician, and an Administrative
Assistant. They work together to ensure your satisfaction with your
home dialysis program, and continued improvement in your quality of
life.
The Dialysis
At Home team works in cooperation with your doctor and health plan to
coordinate services, identify problems, and work out solutions. By
focusing on your specific needs, they can help you achieve health levels
that will give you greater freedom. That way, you can enjoy more productive
time both on and off dialysis. Working together with you, your partner,
and your family, the Dialysis At Home team makes the delivery of your
care easier and more effective.
- Doctor (Nephrologist/Kidney Specialist)
Your Nephrologist (kidney doctor) is responsible
for your care, including orders for your dialysis treatment at home,
changes in your dialysis treatment regimen, and medical evaluation
of your health outcomes. He or she will collaborate with the team
to help you achieve and maintain the best possible health.
- Registered Nurse/Dialysis Care
Manager
A Dialysis Care Manager is available 24
hours a day, seven days a week, to help manage your home care and
direct the other members of the Dialysis At Home team. The Dialysis
Care Manger is a Registered Nurse who works as directed by your doctor
to link you with healthcare providers and community resources. The
Care Manager coordinates your healthcare at home and serves as a resource
for your questions and concerns. He or she also helps identify community
resources that may be of value, provides and coordinates your health
education and support, and helps develop your personal Plan of Care.
- Home Caregiver
The Home Caregiver provides hands-on clinical
assistance for your home dialysis treatment. He or she is either a
qualified dialysis nurse or an experienced client-care technician
who has received specialized training in delivering dialysis treatments,
and caring for people with chronic kidney disease. All Home Caregivers
are supervised and evaluated four times a year by a qualified Registered
Nurse or Dialysis Care Manager in the home setting.
- Renal Social Worker
Another important member of the Dialysis
At Home team, the Renal Social Worker is there to help you manage
the feelings that you, your partner, and your family may have about
your medical condition and your dialysis treatments. He or she can
help you find community and economic resources, and provide individual,
family or group support. The Renal Social Worker will also assist
you in developing a rehabilitation plan. This plan may include physical
therapy, work rehabilitation, and employment counseling. Other support
services can include identifying community and economic resources,
assisting with Medicare/Medicaid issues, providing individual and
family counseling, and taking part in ongoing education and support.
- Renal Dietitian
The Renal Dietitian has training and experience
in helping people with chronic kidney disease follow a healthy diet.
Renal Dietitians use nutritional guidelines and educational materials
to help people improve their nutrition and support better health.
The Renal Dietitian will evaluate your current eating habits, nutritional
requirements, and clinical outcomes. Then, he or she will contact
you, your partner, and your family, to provide healthy suggestions
and recommendations for your diet, including healthy recipes. Nutrition
assessments, nutrition therapy intervention, counseling, and correction
of nutritional deficiencies are all part of your home care program.
- Chief
Technician/Equipment Technician
The Chief Technician/Equipment Technician
coordinates all ordering and delivery of dialysis equipment and supplies.
He or she is responsible for the initial technical and safety assessment
and setup, as well as the calibration of your hemodialysis and water
equipment. Once you have your equipment, the Chief Technician will
help you evaluate the layout of your home to determine the safest,
most convenient place for dialysis treatment. In addition, the Chief
Technician manages an ongoing preventive maintenance program for all
equipment, water quality, and bacterial testing, and supports technical
processes for emergency and problem situations. The Chief Technician
is available during all client home treatments.
- Administrative Assistant
As a member of the Dialysis At Home team,
the Administrative Assistant helps coordinate your supplies and equipment,
and will assist you with any scheduling requirements you may have.
In addition, the Administrative Assistant can answer your questions
about the administrative aspects of your treatment, including insurance
coverage of home hemodialysis services, equipment, and supplies.
