Manager, a Home Caregiver, a Home training Nurse, a Renal Social Worker, a Renal Dietitian, and an Equipment Technician. They work together to ensure your
satisfaction with your home dialysis program, and continued improvement in your quality of life.
The Dialysis At Home team works in cooperation with your physician and health plan to coordinate

- Medical Director
Doctor responsible for overseeing medical care provided in the facility.
- Doctor (Nephrologist / Kidney Specialist)
Your Nephrologist (kidney doctor) is responsible
for your care, including orders for your dialysis treatment regimen, and medical evaluation of your health outcomes. He or she will collaborate with the team to
help you achieve and maintain the best possible health.
- Administrator
Responsible for managing all aspects of facility operations, including
quality of care, delivery of services, and adherence to all aspects of local, state and federal laws.
- Director of Nursing
Registered Nurse (RN) responsible for nursing care and technical
services. In charge of patient care staff, technical staff, social workers and dieticians. Sees that policies and procedures are carried out correctly.
- Clinical Coordinator
Registered Nurse (RN) available 24 hours a day, seven days a week, to
help manage your home care, serves as a resource for your questions and concerns. Oversees staff and works closely with your Nephrologist to implement your care.
- Training Nurse
Registered Nurse (RN) responsible for training patients and partners in the
area of Peritoneal Dialysis and Short Daily Dialysis (Nxstage).
- Home Caregiver
The Home Caregiver provides hands on clinical assistance for your home
dialysis treatment. He or she is either a qualified dialysis nurse or an experienced client-care technician who has received specialized training in delivering
dialysis treatments, and caring for people with chronic kidney disease.
- Renal Social Worker
Available for counseling; helps you get financial aid and social
services, and works with you to explain health insurance coverage and find travel services.
- Renal Dietitian
Works with you to develop a diet plan; teaches you and your family the
importance of diet, proper food selection, and how to adjust the diet plan to meet your needs.
- Chief Technician
Responsible for maintenance and repair of water treatment equipment,
medical equipment and other technical systems to ensure safety of patients and staff. Coordinates all ordering and delivery of dialysis equipment and supplies.
Also will help you evaluate the layout of your home to determine the safest, most convenient place for dialysis treatment.
- Supply/Delivery Technician
Works with Chief Technician while also packaging initial and
monthly patient orders. Verifies orders prior to delivery to ensure efficiency and adequate supplies in your home. Communicates with patients to coordinate
deliveries.
- Billing Coordinator
Will assist you and your family with billing issues and concerns.
Works with you and your insurance company to ensure that you understand your full insurance benefit plans.
